2 years old must attend a minimum of two days weekly, 3 – 6 years old must attend a minimum of three days weekly

Parental Agreement

  1. I wish to reserve a space for my child in the Millbrae Nursery School Preschool Program. Attached is a check for my non-refundable registration fee of $100. Please obtain a receipt for cash payments.
  2. I understand that first and last months tuition and fees must be paid prior to my child starting. (unless arrangements have been made for a payment plan, please speak with the front office)
  3. Four (4) weeks written notice must be given when withdrawing from Millbrae Nursery School.

  4. I am interested in volunteering as a school Board Member.

    NO - I am not able to serve on the board of directors but understand I will have a school job that I must choose from a list of available jobs and will be responsible to fulfill for the entire school year.

  5. I agree, I am able bodied, I will work/participate in the classroom one day per week.

    NO- I will not work in the classroom and will pay the buyout fee of $300/month.

    Sibling spot for a younger child during your workday, (under 2 years of age). Siblings 2 years and older will be billed extended care rate for attending of the family workday.

  6. I understand that Millbrae Nursery School is a Parent Co-operative pre-school and relies upon the participation of the enrolled families to operate. Parents are to participate unless they have elected to buy out of certain obligations (see fees below).
  7. I agree to have all paperwork filled out and submitted before the first day of school.

Please initial & sign above and return application with your registration fee of $100, 1st & ½ last month tuition due prior to first day of attendance.

COVID- Illness:  As with any public or school event, participation in these events includes possible exposure to and illness from infectious diseases including but not limited to Influenza, RSV, MRSA, COVID-19.  While rules and personal discipline may reduce this risk, the risk of serious illness and death does exist.  If not limited to Influenza, RSV, MRSA, COVID-19. If you are no longer able to attend the events, or would like to change your participation to virtual, please contact us and we will be happy to talk with you.
